Added subtle text spacing hints to make <code> fragments stand out more (with some font selections, it was very hard to see the difference between <code> and normal text). Also removed "div.usage" which does not occur in the HTML anymore.

Added "dt.toc", used by "index.html". Override indenting in "p.version", to make sure border groove extends entire line. And some minor tweaks.

Removed "hr" because that is not part of XHTML Basic. Added some explicit color to make sure it looks fine when people use another default style sheet than black-on-white.
