#2 push some patches upstream? Clarify patches in CHANGES and README?

Several suggestions: * For using this the README should refer to this repo, not the original one * Most patches I can see seem like reasonable things to send upstream (I could send them if you don't want to use GitHub) * It would be good to list all the changes in a special top section of the CHANGES doc
